Large employer applications for COVID-19 Leave Support Scheme
If you're a large employer (eg 100 employees) applying for the COVID-19 Leave Support Scheme, you can enter your details into a spreadsheet and send this to us.
The Leave Support Scheme is for businesses with employees who can’t work from home and either:
- has COVID-19
- is caring for a dependant who has COVID-19, or
- is at risk of severe illness from COVID-19, or has a household member who is, and have been advised by a medical practitioner to self-isolate.

Only use this process if you’re applying for 100 or more employees. If you’re applying for less than 100 employees you must use our Employers online form.
Before you apply, make sure:
- the employees you're applying for meet the eligibility criteria set out in the declaration.
- these employees cannot work from home.
- you've checked the criteria on the 'Who can get the COVID-19 Leave Support Scheme' page and your business and your employees meet the criteria.
- your business details match those held by Inland Revenue, especially your business IRD number.
- you know whether your employees usually work 20 hours or more a week, or less than 20 hours a week (this is their "employment type").
- you’ve agreed in writing with your employees on casual contracts or with employees who work more than their contracted hours, what their hours would usually be during the period you're applying for the Leave Support Scheme.
- you've read the declaration and understand your obligations for this payment.
- you’ve prepared and retained evidence to support your application that shows your employees must self-isolate. Evidence could include, but isn’t limited to, employee leave records. We may ask to see this evidence.
- the information for your employees is correct, you've told your employees you're applying for them, and you have their consent to share their information.
Information you need to enter in the spreadsheet
Please make sure the details you provide match those held by Inland Revenue, especially your IRD number so we can process your application quickly. Do not change the CSV file format or layout. If you do, it will delay your application.
The information you'll need to enter in the CSV spreadsheet file is your:
- business IRD number (this could include your partnership IRD number or company IRD number)
- IR customer name (this is the name that's linked to your business IRD number)
- New Zealand Business Number (NZBN) if you have one - this is optional
- business address
- contact name, email address and mobile number
- bank account number
- your employee details (include full-time and part-time employees in the same application):
- name
- date of birth (enter as dd/mm/yyyy)
- IRD number
- employment type (enter 'full-time' if they usually work 20 hours or more a week, or 'part-time' if they're working less than 20 hours a week).

When you complete the CSV file, it's important to follow the tips below which will help us process your application faster:
- The first 2 rows are prefilled for you, so you don't need to do anything but please don't remove these rows
- The contact number you provide must be a New Zealand number
- Please do not use special characters, for example é
- When you enter your bank account details, please use hyphens not full stops eg 01-1111-1111111-11
- Save the file (keep it as a CSV file) and rename it with your business name
- Read the COVID-19 Leave Support Scheme declaration
- Copy and paste the following text into an email to confirm you agree to the declaration: I confirm that I have read and understood the declaration.
- Attach the CSV file to your email
- Send it to us at COVID19_Employer_Support@msd.govt.nz
