Residential care administrator notifications

If you're an administrator of a rest home or private hospital where one of our clients is in long-term care, you need to tell us about changes in their situation.

Changes you need to tell us about

You need to tell us if a client either:

  • moves into your facility long-term
  • moves to another facility
  • moves into a public hospital and is not returning to your care
  • returns to the community and is not returning to your care
  • passes away.

You need to tell us within 24 hours after the change happens.

What happens next

After you complete the form, our staff will process this change within 5 working days.
